If you had to pay for these tests in Canada, I'm sure patients would be sent for as many as possible like in China. However, there is no need for an early ultrasound unless it would help to pinpoint your expected due date(because either you don't know you last menstrual period or your period is irregular). An ultrasound is also required if you have some some complication like bleeding, pain, severe vomiting, previous history of complicated pregnancies. An ectopic pregnancy causes pain and bleeding, so if you have it you'll end up in hospital. It's hard to screen for ectopic pregnancies because transabdominal ultrasounds may not see a fetus until 6-8 weeks and this is when ectopics would present anyways. A transvaginal ultrasound can examine a fetus at as early as 4 weeks, but it is invasive and causes discomfort. Therefore it might be reasonable to screen people at very high risk for ectopics, if you have a history of previous ectopic, sexually transmitted diseases (pelvic inflammatory disease), uterine abnormalities (endometriosis, previous surgeries), IVF, etc. ; s p+ L2 c9 p. t0 {5 d/ Z8 F

The standard ultrasound for examining fetal structures is at 18-20 weeks because it is not possible to see all the organs prior to this time. An early ultrasound, aside from identifying dates, leads to a lot of ambiguitiy. If you are feeling absolutely fine, but you are told there is something unusual about the ultrasound and it is not possible to say what the problem is, what the hell would you do with that information. You'll end up stressed and worrying about your pregnancy. Therefore an early ultrasound should only be requested if there is a reason, like Pearlo above, who has severe vomiting.

2) folic acid$ W% ?, U2 D: p- k5 J' X
If your age is >34 then you need higher doses of folic acid (5mg). Also Northern Chinese people are apparently at higher risk for neural tube defects so they should be given 5mg. If you've had other problems with previous pregnancies such as birth defects, miscarriages, you should take 5mg. It should be taken prior to becoming pregnant and for the first trimester (although really it should be just 4 weeks after conception). Multivitamins are needed for the first trimester, but is not absolutely necessary for 2nd/3rd trimesters unless you don't eat a balanced diet, although most mothers take it for the whole duration of the pregnancy anyways. |
